By accessing or using the EncorVita™ Health website at encorvita.com (the “Site”), you agree to be bound by these Terms of Service (“Terms”) and our Privacy Policy. If you do not agree to these Terms, please do not use the Site.

EncorVita™ Health is a healthcare education and practice expansion company. The Site provides information about our services and allows licensed healthcare providers, aesthetic clinics, medical spas, and other healthcare businesses to express interest in our pilot program by submitting a contact form.

The Site is not a patient portal and does not offer medical advice, diagnosis, or treatment. Nothing on the Site constitutes a patient-provider relationship. We are not a HIPAA covered entity.

3. Permitted Use

You may use the Site only for lawful purposes and in accordance with these Terms. You agree not to:

IN NO EVENT SHALL ENCORVITA™ HEALTH’S TOTAL LIABILITY TO YOU FOR ALL CLAIMS EXCEED ONE HUNDRED DOLLARS ($100.00).
